Lower eyelid laxity

The right lower eyelid has become lax causing the old prosthetic eye to drop relative to the companion eye.

Diagram surgery for lower lid laxity

Lower eyelid laxity is most commonly addressed by shortening the lateral canthal tendon and reattaching it to the periosteal lining of the orbital rim. This tightens the orbicularis muscle and raises the lower eyelid. Sometimes shortening the lateral canthal tendon does not raise the eyelid sufficiently and in this case both the medial and lateral canthal tendons are shortened.
